CCACs were established by the
Ontario Government to simplify and improve the delivery of community
long-term care services. There are six independent, non- profit CCAC
organizations in Toronto, each governed by a volunteer board of
directors from the respective communities. They represent a single point
of access in each area for in-home services such as: nursing,
physiotherapy, social work, speech, occupational therapy, nutritional
counselling, personal support, and for assessment and placement for
long-term facilities (nursing homes). The CCACs are also a resource for
information and access to a wide range of community services such as:
meals, programs, transportation, or friendly visiting.
The goal of the CCACs is to provide in-home services to
ensure that the recipient can continue to live independently at home if
he or she is sick or disabled. CCACs work in partnership with the
recipient of the services, that person’s physician and the family, to
discuss health and support needs and goals, and to determine what is the
best course of action to take in order to meet those goals.
