Complex Continuing Care and
Long Term Care services are accessed via CCAC's,
Acute Care Hospitals, The Regional Geriatric Program, the Palliative
Care Program, and community physicians.
A)
Acute Care
Specializing in Multiple
Sclerosis Hospitalization having a short course of treatment.
B)
Complex Continuing Care
(formerly Chronic Care)
Care in facilities governed by the Public Hospitals Act, offering
hospital-based care required by persons whose condition is medically
unstable and/or requires skilled, technology-based continuing or
intermittent care.
C)
Respite Care
Provides care for a person with physical challenges, in order for the
care-giver to have an interval of rest or relief.
D)
Short Stay / Rehabilitation
These services are accessed via Placement Coordinating Services, acute
care hospitals, Regional Geriatric Program, Palliative Home Care
Program, and community physicians:

Baycrest
Centre for Geriatric Care:
The Behavioural Neurology Program is a short-term
assessment and treatment program for people suffering disturbances
in cognitive function and behaviour due to brain dysfunction.
The Geriatric Assessment and Treatment Unit (GATU) provides
diagnostic and functional assessments for short-term treatment, for
frail, elderly clients who are experiencing medical illness and
recent function decline.
The Palliative Care Unit of Baycrest Hospital provides 24-hour pain
and symptom control, combined with emotional and spiritual support
to terminally ill patients.

Providence
Centre:
Programs include complex continuing care, ortho-neuro
rehabilitation, palliative care, complex medical care, geriatric
assessments, and cognitive support.

West Park
Healthcare Centre:
The Neurological Day Program (NDH) offers an individualized day
treatment program for individuals with MS. Clients may attend two
days a week in an active, goal-directed and client-focussed
program. This program is for clients managing their activities of
daily living (with or without assistive aids) in the community.
Clients must require a minimum of three professional services
(e.g. Occupational Therapy, Physical Therapy, Nursing,
Speech-Language Services, or Recreational Services) to be eligible
for this program.
The Regional Multiple Sclerosis Short Stay Service (RMSSS) offers
a six to eight week in-patient Short Stay Service for residents of
the Toronto Area, and North Central Ontario. This client-focussed
service is designed to meet the needs of persons with MS who are
experiencing serious difficulty managing in the community, and
require the time-limited services of a trans-disciplinary team of
care-givers, to meet their complex needs.

Toronto
Rehabilitation Institute:
Both outpatient and inpatient rehabilitation services are provided
at several of the centres in Toronto. Access to all rehabilitation
professionals is provided, including physical and occupational
therapy, social work, nursing, and speech- language pathology. A
variety of programs is offered from an outpatient health promotion
program through day hospital services, short and long term care,
inpatient, rehabilitation, and respite care.
